Primary care physicians are those MDs or DOs specializing in family practice, general practice, pediatric medicine, or internal medicine. Data are from the Centers for Medicare and Medicaid Services (CMS) Physician and Other Supplier 2012 Public Use File (PUF), which was released in April 2014, and from the 2013 CMS Physician Compare National Downloadable File.

Physician directory and location data are from the Centers for Medicare and Medicaid Services (CMS) Physician and Other Supplier 2012 Public Use File (PUF), which was released in April 2014, and from the 2013 CMS Physician Compare National Downloadable File. Additional data analysis was performed by CARES.
